1. Completed and signed Lenders Access application ( ERC - Get Started "Form" )

2. IRS 941s (Quarter 2 2020 through Quarter 3 2021 )

3. State Payroll Tax Returns (Wages and Contributions) (Quarter 2, 2020 through Quarter 3, 2021 )

4. PPP Loan Information for 2020 and 2021 (Date Received, Amounts and Forgiveness Letters)

5. Copy of a VOIDED company check.

6. List of family members, if any, on payroll for any given quarterly periods

Items needed to do the Funding:

1. Completed and signed Lenders Access application  (Completed from ABOVE)

2. IRS 941s (Quarter 2 2020 through Quarter 3 2021 AND Quarter 3 2022 through Quarter 4 2022 )

3. IRS 941Xs (amended return)(Quarter 2 2020 through Quarter 3 2021 )

4. ERC calculations to support the ERC filing

5. Copy of the most recent year’s tax return

6. Most recent business bank statements (3 Months /all pages)

7.  P&L 2020 & 2021
